- Mantoux TB (Once admitted, yearly TB Testing)

- Previous Group Home Address, Phone Number, and Providers Name
- Any Knowledge of Deviant or Sexual Behavior (Agency's Knowledge of Sexual Offenses)
- Psychological Diagnosis
- Current Diagnosis
